Our answering service will be discontinued shortly and cannot be used to request medical records or prescriptions.

I will continue to care for patients in local nursing homes. I will continue to make regularly weekly visits to the nursing homes and see my patients as previously. Patients who have an urgent medical need should speak to a member of the nursing home staff. I will continue to be available to the staff between weekly visits for urgent and/or emergent medical needs.

No further prescription refills, referrals, insurance authorizations or medical equipment authorizations will be completed for office patients. The response will be “this patient is no longer under my care”.

For those patients who have requested medical records already, you will receive a call from myself or the staff when they will be available for you to pick up. If you fail to pick them up I will have them, but the convenience of picking them up a the office will disappear sometime in mid December 2024. If you will need them to be mailed the charge is $10.00 per patient medical record.

Any standard medical record release from any physician’s office is acceptable to use. Here is our form if you wish to use it. Please be aware that to be useable the form must have full patient name, patient date of birth, patient social security number, patient daytime phone number, and must be signed by the patient, the patient’s parent or guardian, or a previously designated adult who can obtain the records (we must already have this designation in our files for it to be useable).

The completed form can be mailed to us at the new business office address. It generally takes about a week to produce the flash drive with your medical records on it. The requests are done in the order they are received.
